I confirm that I have read and understood the following statements and that as a spectator at the Vitality Netball Superleague 2021 I will comply with the following:
- I understand that under current government guidelines all spectators (unless exempt or when eating and drinking) will be required to wear a face covering at all times whilst at the venue (note that proof of exemption is required)
- I understand that under current government guidelines social distancing must be maintained at all times
- I understand that I should regularly wash my hands whilst at the venue using the sanitiser stations provided
- I understand that I will be required to print and bring my e-ticket(s) with me to the venue (or show my e-ticket(s) on my smartphone) and that failure to do so will mean that I will be denied entry to the venue
- I understand that I will be required to bring proof of identity with me (ie driving license or passport) and that failure to do so will mean that I may be denied entry to the venue
- I understand that I am required to correctly input the details of all attendees within my group (where applicable) at the time of booking
- I understand that tickets are non-transferable and non-refundable
- I understand that match times and the competition schedule are subject to change
- I understand that I am permitted to bring a reasonable amount of food and drink into the venue with me but acknowledge that the following principles apply:

- Drinks bottles must not exceed 500ml in size
- Alcohol is not permitted
- Glass or metal containers are not permitted
- I will be required to perform a sip test upon entry
- I understand that I must not travel to the venue if I am required to self-isolate by NHS Test and Trace
- I understand that I must not travel to the venue if I feel unwell or display any COVID-19 symptoms, including:

- A high temperature
- A new, continuous cough
- A loss of, or change to my sense of smell or taste
- I understand that I should plan my journey to the venue in advance to avoid busy routes and times, and that I must wear a face covering if travelling via public transport
- I understand that spectator doors will upon 75 minutes prior to the first centre pass of the day
- I understand that there will be health screening and additional COVID-19 measures and processes in place upon entry to the venue, including being required to scan the NHS QR code
- I understand that additional COVID-19 measures and processes will be in place around the venue and that I will need to observe the signage in place and follow the direction of the stewards at all times
- I understand that I will need to behave appropriately at all times and that singing and shouting will not be permitted
- I understand that I should remain in my seat at all times whenever possible
- I understand that should I need to leave my seat for any reason, I should wait for a time when the gangway is clear and ensure that when moving past other spectators to and from my seat that I must avoid face to face contact with others
- I understand that should I begin to feel unwell onsite or display any COVID-19 symptoms that I should inform a member of the stewarding team immediately
- I understand that following the conclusion of the final match I may be required to remain in my seat until instructed to leave by the stewards
- I understand that I am required to pass on a copy of the Code of Conduct to all other attendees in my group who are also all required to comply accordingly
- I understand that despite all the COVID-19 protocols in place, there is still a risk that I may catch COVID-19 or be required to self-isolate as a result of attending the event
I understand that failure to comply with any of the above statements will be treated extremely seriously and may result in sanctions, including expulsion, bans, or reporting to the police in line with Government requirements.
